This survey includes 15 questions that solicit student feedback on school culture and climate. Appropriate for grades 6 through 12, it includes questions on safety, bullying, friendship, academic expectations and adult/student interaction. It can be used to gather student input on school policies, climate, and related issues, and in the promotion of student-focused leadership on school climate issues.
